TIRZEPATIDE

Tirzepatide is a synthetic peptide based on the native GIP sequence, modified to incorporate dual agonist activity at both the glucose-dependent insulinotropic polypeptide receptor (GIPR) and the glucagon-like peptide-1 receptor (GLP-1R), with a C20 fatty diacid moiety enabling albumin binding for once-weekly dosing. Its mechanism involves biased agonism toward the cAMP signaling pathway at both receptors, with combinatorial activation producing synergistic effects on glucose-stimulated insulin secretion, glucagon suppression, and gastric emptying delay that exceed outcomes observed with selective GLP-1R agonists alone. Phase 3 SURPASS clinical trial data demonstrated significant HbA1c and body weight reductions across dose levels, with tirzepatide receiving FDA approval in 2022 and currently under investigation for obesity, heart failure, sleep apnea, and non-alcoholic steatohepatitis.
