LL-37 is a 37-amino acid cationic amphipathic peptide and the sole human member of the cathelicidin family, generated by proteolytic cleavage of the hCAP18 precursor protein in neutrophils, epithelial cells, and other immune-competent tissues. Its antimicrobial mechanism involves direct disruption of negatively charged microbial membranes through pore formation and membrane permeabilization, with activity documented against Gram-positive and Gram-negative bacteria, fungi, and certain enveloped viruses. Beyond direct antimicrobial function, research has characterized LL-37 as a pleiotropic immunomodulator capable of modulating formyl peptide receptor (FPR2) signaling, promoting chemotaxis, stimulating angiogenesis, facilitating wound re-epithelialization, and exerting both pro- and anti-inflammatory effects depending on the local cellular microenvironment.
